Effect of intravenous lornoxicam for postoperative patient controlled analgesia with morphine on cytokines

Abstract

Objective:To assess the effect of intravenous lornoxicam for postoperative patient-controlled analgesia with morphine on cytokines.Methods:69 patients(ASA I~Ⅱ)undergoing elective upper abdominal procedure were randomly divided into three groups: morphine alone 0.25 mg·mL-1 at the end of the procedure(group M),adding lornoxicam 8 mg at the end of the procedure and at the 8 hours post operation to the morphine regimen(group L2),or adding lornoxicam 8 mg at the end of procedure and at the 8 hours and 16 hours post operation to the morphine regimen respectively(group L3),VAS were used to evaluate the analgesic response.Blood serum concentrations of CRP,IL-2 and IL-6 were measured before anesthesia and at postoperative 24 h and 48 h.Results:Patients in three analgesic groups did not show significant differences in the VAS values(P0.05).There was significant increase in the level of CRP in all groups comparing with the level before anesthesia(P0.01).CRP in group L2 and L3 was lower than that in groups M at postoperative 24 h(P0.05).At postoperative 24 h,the level of IL-6 in group M was higher than the level in groups L3(P0.05).At postoperative 48 h,The levels of IL-2 in all groups were decreased(P0.05),but there were no significant differences in all of the groups(P0.05).Conclusion: The dual add-on postoperative intravenous administration of lornoxicam to the morphine regimen can provide more efficient effect in maintaining the balance of pro/anti-inflammatory cytokines,alleviating inflammatory stress response.
