Effect of Intravenous Injection of Cedilanid and Metoprolol on Patients with Acute Left Heart Failure and Atrial Fibrillation

Abstract
Objective To research the effect and safety of intravenous injection of cedilanid and small does of metoprolol tartrate in patients with acute left heart failure and rapid ventricular rate of atrial fibrillation.Methods Seventy-six patients with acute left heart failure and rapid ventricular rate of atrial fibrillation treated between June 2005 and February 2012 were divided into control group(n=39) and treatment group(n=37) at random.The control group was administered with cedilanid,and the treatment group with cedilanid and small does of metoprolol tartrate by intravenous injection.Systolic blood pressure,heart rate,respiratory rate,pulse oxygen saturation(SpO2),dyspnea,urine amount and the time from beginning of treatment to improvement of acute left heart failure were monitored as soon as treatment began.Results There was no significant difference in basic data such as systolic blood pressure(P=0.525),heart rate(P=0.246),respiratory rate(P=0.390) and SpO2(P=0.482) at the beginning of treatment between the two groups,and during the whole treatment process,no obvious difference was seen in systolic blood pressure and 2-h urine amount(P=0.264) between the two groups.Obvious decreases took place in heart rate in the treatment group after the treatment began(P=0.000),and the time from beginning of treatment to improvement of acute left heart failure in the treatment group was significantly shorter(P=0.003).No patient died during the test.Conclusion Without contraindication of metoprolol,under strict monitoring of lung rals and SpO2,cedilanid combined with small dose of metoprolol by intravenous injection can effectively,quickly and safely control the ventricular rate and improve the acute left heart failure in patients with rapid ventricular rate of atrial fibrillation whose heart function grades of New York Heart Association(NYHA) are Ⅰ to Ⅱ before onset of acute left heart failure.
