CLINICAL ANALYSIS OF OVARIAN MALIGNANT TUMOR IN CHILDREN AND ADOLESCENTS:RETROSPECTIVE STUDY OF 48 CASES DURING 25 YEARS

Abstract

[Objective]To explore the clinical diagnosis and therapy features of ovarian malignant tumor in children and adolescents and analyze the influential factors of prognosis.[Methods]48 cases(girls aged less than 18 years old )of ovarian malignant tumor in our hospital from 1980 to 2005 were reviewed. According to histological diagnosis,the cases were divided into 3 groups:germ cell tumor group,sex cord-stromal tumor group,and epithelial carcinoma group. Age,histological classification,symptoms,stage,surgery and chemotherapy strategies and follow up results were analysed.[Results]The age of the cases ranged from 4 to 18 . The most common histological type was malignant germ cell tumor,with 32 cases(66.7%). Three more common symptoms were:abdominal mass,abdominal pain and distention. 39 cases(81%)were in stage Ⅰ-Ⅱ. Five-years survival percentage was 72.8% and 76.2% in preserving productive function operation group and cytoreduction group respectively. The difference between two types of operation wasn't statistically significant. Stage and histological type were the important influential factors.[Conclusion]To make an earlier diagnosis,we should pay more attention to the characteristics of such malignant tumors,meanwhile adolescents should be mass screened. Preserving productive function operation and chemotherapy are applicable for the girls of stage Ⅰ-Ⅱ. The patients with ovarian malignant tumor have more overall survival advantages.
