Clinical significance of the combined detection of Brain Natriuretic Peptide and High-Sensitivity C-Reactive Protein in prognosis of Chronic Heart Failure

Abstract
Objective To evaluate the long-term clinical value of brain natriuretic peptide(BNP) and high-sensitivity C-reactive protein(hs-CRP) in patients with chronic heart failure(CHF). Methods The plasma BNP and hs-CRP lev els in 98 CHF patients were measured and compared with 48 subjects as normal con trol.These patients were followed up for a mean period of 1 year to observe the occurrence of cardiac events. Results Compared with control group,BNP and hsCRP levels in CHF patients change significantly.Besides,for CHF patients,with the NYNA grades raises,BNP and hs-CRP levels increase progressively.During fo llow-up period,CHF patients with cardiac events have higher BNP and hs-CRP lev els than patients without cardiac events.When BNP and hs-CRP were used to predi ct cardiac death of CHF,the areas under the receiver operating curve(ROC) was 0.906 and 0.941 respectively.For predicting cardiac death in patients with CHF, BNP cutoff point of 323.5 pg/ml had a sensitivity of 79.3%,a specificity of 85.5%.At the same time,hs-CRP cutoff point of 8.5mg/L had a sensitivity of 75.9 %,a specificity of 85.7%.Kaplan-Meier cumulative survival analysis showed t hat the survival rate of patients in group of BNP323.5 pg/ml and hs-CRP8.5 mg /L was much higher than the other groups(P0.01). Conclusion BNP and hs-CRP levels were significantly higher in patients with CHF.Monitoring of plasma BNP levels has important clinical significance to judge the prognosis in patients with CHF.
