Abstract
[ 0bjective] To evaluate the clinical effects of sildenafil on severe pulmonary hypertension after cardiac surgery. [Methods] 16 cases of severe pulmonary hypertension after cardiac surgery were treated with sildenafil from November 2005 to November 2008. All these cases were diagnosed to be severe pulmonary artery and given discontinuous oxygen uptake and general treatments including intravenous prostaglandin E1 before the use of sildenafil. Digitalis was stopped before the surgery and deformity malformation surgery was performed to them. Pulmonary artery pressure, systolic systemic blood pressure, systolic pulmonary artery pressure / systolic systemic blood pressure (PP / Ps), The blood oxygen saturation, arterial partial pressure of oxygen and inhale an oxygen density were measured before and every hour after adding sildenaftl. [Results] At the one hour after adding sildenafil, the patients pulmonary artery pressure decreased significantly but no adverse effects showed on the systematic artery pressure.PaO2 / FiO2 of all cases improved respectively.One or two days later.the patients hemodynamics were stable and dosage of prostaglandin E1 decreased.16cases stopped use of breath machine and were discharged safely. [Conclu-sion] Sildenafil is a highly selective vasodilator, which can be used in the treatment of pulmonary hypertension after cardiac surgery.
