Value of MELD score combined with serum cholesterol in predicting prognosis of patients with severe hepatitis

Abstract

Objective To assess the value of the model of end-stage liver disease(MELD) score combined with serum cholesterol in predicting prognosis of patients with severe hepatitis.Methods A total of 105 severe hepatitis patients were divided into two groups of A(survival) and B(death) according to the results of followed-up for 3 months.Serum total bilirubin(TBil),creatinine(Cr),international normalized ratio(INR),cholesterol(CHOL) were measured,based on which MELD scores were calculated.The ROC curve was used to evaluate the capability and the best threshold of MELD score combined with serum cholesterol for predicting the prognosis of patients with severe hepatitis.Results The higher was the MELD score,the higher mortality was in patients with severe hepatitis.The serum TBil,INR and MELD score were higher,serum CHOL was lower,in group B than those in group A(P0.05).The area under ROC curve of MELD score and CHOL were 0.825 and 0.785 for predicting the 3-month prognosis,respectively.To predicte death in patients with severe hepatitis,the best the critical value of MELD score was 25 points and that of serum CHOL was 1.65 μmol/L.The specifity and sensitivity of MELD score combined with CHOL were 92.52% and 90.23%.Conclusion MELD score and CHOL were good indicators for predicting the short-term prognosis in patients with severe hepatitis.
