Abstract

目的 探讨无创双水平气道正压通气(BiPAP)对重度充血性心力衰竭的作用.方法 选择42例经常规方法治疗无效的重度充血性心力衰竭患者进行BiPAP,记录临床症状体征、Boston 心衰积分、彩色多普勒二维超声心动图及血气分析等指标监测.结果 42例重度充血性心力衰竭患者抢救成功37例次,抢救成功率8 8.1%.治疗4 h后生命体征稳定、心衰减轻,PaO2和SaO2明显提高(P<O.01);3 d后心脏射血分数明显增加(P<O.01).结论 无创双水平气道正压通气可改善心功能,减轻肺水肿,迅速提高动脉血氧含量,是治疗心源性肺水肿的一种有效、简便、无创而实用的治疗方法。
