The effect of continuity of nursing intervention on controlling elderly patients with chronic heart failure and their self-care

Abstract

Objective To explore effect of the continuity of nursing intervention on controlling elderly patients with chronic heart failure.Methods A total of 110 chronic heart failure patients in our hospital between January 2012 and December 2012 were divided into intervention and control groups,55 cases in each group.The control group patients were received the conventional loop medical care,the intervention group patients were received regular telephone follow-up and outpatient follow-up within six months.After 6 months,the readmission rate,number of day,mortality and self-care scores of the two groups were analyzed.Results The readmission rate,number of readmission and total number of days of intervention group were lower than those of control group (x2 =4.475,t =2.689、-2.872) ; the ability of patients'self-care was better than that of control group (t =3.346) ; there was no statistical differences in mortality rate between the two groups (x2 =0.853).Conclusions The continuity of nursing intervention can effectively control the development of elderly patients with chronic heart failure,improve the ability of patients' self-care.
