Abstract

Objective To investigate the effects of hemodialysis combined with blood perfusion on insulin resistance, micro inflammation and nutritional status in patients with end-stage diabetic nephropathy. Methods Ninty-six cases of end-stage diabetic nephropathy were selected, and were divided into control group and observation group, with 48 cases in each group. All patients were given three times hemodialysis in one week. The control group received one time hemodialysis combined with hemofiltration on this basis, and the observation group received one time hemodialysis combined with blood perfusion in addition. The indexes of insulin resistance, micro inflammation and nutritional status were compared and analyzed before and after treatment between the two groups. Results Before treatment, there was no significant difference in the indexes between the two groups (P>0.05). After treatment, the levels of C-reative protein (CRP), tumor necrasis factor-α (TNF-α) and interleukin-6 (IL-6) decreased significantly in the two groups, and those of the observation group was significantly lower than those of the control group (P 0.05). Conclusions Patients with end-stage diabetic nephropathy have different degrees of insulin resistance, micro inflammation and malnutrition. Hemodialysis combined with blood perfusion therapy can effectively reduce insulin resistance, eliminate the inflammatory factors, improve the nutritional status, and improve the overall treatment effect. Key words: Hemodialysis; Hemoperfusion; Insulin resistance; Inflammation; Malnutrition
