Abstract
Objective To investigate the clinical efficacy of intravitreal injection of Lucentis combined with panretinal photocoagulation and trabeculectomy for neovascular glaucoma (NVG). Methods Thirty-seven eyes of 37 NVG patients who received treatment in our hospital were randomly divided into 2 groups. Nineteen patients were treated with intravitreal injection of Lucentis 0.5 mg, combined with panretinal photocoagulation and complex trabeculectomy in group A. Eighteen patients were treated with panretinal photocoagulation combined with transscleral cyclophotocoagulation in group B. Changes of visual acuity, intraocular pressure (IOP), iris neovascular vessels and complications were observed before and after operation in both groups. Results Two to three days after intravitreal injection of Lucentis, iris neovascular vessels disappeared in all patients of group A. Panretinal photocoagulation was completed within 1 month. IOP of 19 eyes were higher than 30 mmHg one month after injection. After complex trabeculectomy, IOP was (11.35±3.28)mmHg (1 mmHg=0.133 kPa) in one week. The IOP were (12.37±5.13)mmHg, (14.45±5.38)mmHg and (16.27±4.28)mmHg respectively in 1, 3 and 6 months. The 18 patients in group B taken transscleral cyclophotocoagulation after panretinal photocoagulation. The IOP were (13.35±3.27)mmHg, (20.48±4.25)mmHg, (24.45±6.27)mmHg and(26.27±7.28)mmHg respectively 1 week, 1 month, 3 months and 6 months after operation. There was statistically significant difference in IOP between the two groups (P<0.05). Neovascular vessels were found on the iris in 2 eyes of group A and in 8 eyes of group B, and there was statistically significant difference between the two groups (P<0.05). Conclusion Intravitreal injection of Lucentis combined with panretinal photocoagulation and trabeculectomy is effective for NVG.
