Effect of dexmedetomidine hydrochloride combined with intermittent ventilation before single pulmonary ventilation on the plasma inflammatory factors and lung function of patients undergoing lung cancer radical surgery

Abstract

Objective To investigate the effect of dexmedetomidine hydrochloride combined with intermittent ventilation before single pulmonary ventilation on the plasma inflammatory factors and lung function of patients undergoing lung cancer radical surgery.Meth-ods A retrospective study was performed on 120 cases of patients who underwent radical surgery of lung cancer from January 2014 to June 2016.Patients were randomly divided into Group A and Group B,with 60 cases in each group.Both groups of patients were per-formed tracheal intubation mechanical ventilation after anesthesia,and the endotracheal intubation was successfully changed to single lung ventilation.In Group A,the non-ventilated side lung was not given any treatment,the non-ventilated side lung was aspirated for 10 seconds by negative pressure aspirator,then open to air,double lung ventilation 5 minutes later,single lung ventilation 5 minutes later;repeated the detailed operation once more after three cycles,then double lung ventilation.Blood samples were collected from the radial artery and the venous blood of the neck at T1(immediate before single lung ventilation),T2(single lung ventilation 0.5 hours),T3 (single lung ventilation 1 hour),T4(single lung ventilation 1.5 hours) and T5(end of operation);blood gas analysis was performed, recording to the alveolar arterial oxygen partial pressure(PA-aDO2) and arterial oxygen partial pressure(PaO2),to calculate the inter-nal rate of the lung(QS/QT). Bypass method was used to monitor the dynamic pulmonary compliance(Cdyn),platform pressure (Pplat) and airway peak pressure(Ppeak).At the same time,IL-6,IL-8 and TNF-α levels were compared between the two groups.Re-sults IL-6,IL-8 and TNF-α levels of both groups at T2,T3,T4,T5were significantly higher than those at T1.The levels in Group B were significantly lower than those in Group A(P<0.05).Levels of PA-aDO2,PaO2and QS/QTat T2of both groups were significantly higher than those at T1.Levels of PA-aDO2,PaO2and QS/QTat T3,T4and T5were significantly lower than those at T2.Levels of PA-aDO2and QS/QTat T2,T3,T4and T5in Group B were significantly lower than those in Group A,PaO2level in Group B was significant-ly higher than that in Group A(P<0.05).Levels of Cdyn in both groups at T2,T3,T4and T5were all significantly lower than those at T1,levels of Cdyn in Group B was significantly higher than that in Group A(P<0.05).Levels of Ppeak and Pplat in both groups at T2,T3,T4and T5were all significantly higher than those at T1,levels of Pplat and Ppeak in Group B were significantly lower than those in Group A(P<0.05).Conclusion In the radical treatment of lung cancer,dexmetomidine hydrochloride combined with intermittent ventilation before single pulmonary ventilation can effectively reduce the release of inflammatory factors and improve the lung function of patients.
