Abstract
Objective To explore the effect of noninvasive positive pressure ventilation(NPPV) on the patients with acute exacerbation of chronic obstructive pulmonary diseases(AECOPD) complicated with type Ⅱ respiratory failure.Methods Thrity-six patients with AECOPD complicated with moderate to severe type Ⅱ respiratory failure were randomly divided into test group(18 cases) and control group(18 cases).Besides conventional treatments,the control group received routine oxygen therapy(2 L/min) and the test group received bi-level positive airway pressure(BiPAP) ventilation.The heart rate(HR),respiratory rate(RR),SpO2% and arterial blood gas parameters(pH,SpO2,PaO2) were recorded before and 2h,24h after treatment.Results After 2h and 24h of BiPAP ventilation,the values of pH,SpO2 and PaO2 improved significantly and the values of HR,RR and PaCO2 decreased obviously in the test group than those before treatment or those in the control group at same stage(P0.05,respectively).Conclusion NPPV can be used as the first choice for the ventilation treatment of AECOPD complicated with moderate to severe type Ⅱ respiratory failure because of its immediate effect.
