Abstract
Objective: To investigate influences of propofol target controlled infusion (TCI)anesthesia or sevoflurane anesthesia on the neuromuscular effect of cisatracurium.Methods: Forty-two ASA(The American Society of Anesthesiologists) class Ⅰ or Ⅱpatients undergoing selective operations were randomly divided into propofol TCI group (group P,n =21)and sevoflurane inhalation group(group S, n=21).All patients were injected with cisatracurium 0.2 mg/kg after anesthesia induction. Patients in group P were maintained with propofol TCI and remifentanil, and those in group S with 1.3 minimal alveolar concentration (MAC)sevoflurane and remifentanil.The train of four (TOF)mode of stimulation was used to monitor neuromuscular blocking variables of onset time,no-response time,the time to recovery of T1 to 25% of control,recovery index and the TOF ratio (TOFr) to 25%. Results: It was found that T0Fr to 25% and recovery index were greater under sevoflurane anesthesia than those under propofol-remifentanil anesthesia (P 0.05). But there were no significant differences in the onset time,no-response time,and T1 25% recovery time between two groups(P 0.05).Conclusion: The propofol TCI-remifentanil anesthesia has little effect on cisatracurium but 1.3 MAC sevoflurane -remifentanil enhanced the neuromuscular effect of cisatracurium.These indicated that if the general anesthesia is maintained with sevoflurane, the dosage of cisatracurium should be reduced properly to avoid the postoperative muscular relaxants residuals.
