Abstract
Objective To evaluate the effectiveness of high-dose immunoglobulin and high-dose methylprednisolone in idiopathic thrombocytopenic purpura(ITP).Methods 101 cases of ITP were randomly divided into 2 groups: Group A,40 patients were treated with intravenous high-dose immunoglobulin[400 mg/(kg·d),for 3-5 days];Group B,61 patients were treated with high-dose methylprednisolone[1.0 g,for 3 days].After half every 3 days reduce to half dose and after 9 days change of prednisone 40-60 mg/d to maintain.Platelet counts were measured every day.Results Compared with before treatment,two treatment methods can significantly improve platelet levels.The total effective rate to enhance the platelet to 100×109/L of high dose of gamma globulin group(A group) and high dose methylprednisolone Dragon group(B group) was 67.5% and 62.3%,the difference was not significant(P0.05).Both the two treatments can significantly improve platelet counts;there was remarkably significant difference(P0.05) in the effective rate(70.4% versus 92.2%),between the two groups,the times of PLT≥50×109/L were(4.88±4.22) days and(5.75±3.79) days;the times of PLT≥100×109/L were(5.3±2.74) days and(6.71±4.36) days,PLT-peak values(×109/L)were(174.48±62.06) and(242.50±134.92) respectively.Conclusion High-dose immunoglobulin was more rapid and effective and superior to high-dose dexamethasone in increasing the platelet count in ITP,and longer in making platelets rise to normal and stable time.
