Differential Diagnosis of Hepatocellular Carcinoma and Hepatic Hemangioma by Contrast-enhanced Ultrasonographic in Patients with Hepatic Cirrhosis

Abstract
Objective To investigate the clinical value of contrast-enhanced ultrasonographic(CEUS) in differential diagnosis of hepatocellular carcinoma and hepatic hemangioma in patients with hepatic cirrhosis.Methods The CEUS manifestation was retrospectively analyzed in 50 patients with lesions in focal hepatic cirrhosis confirmed by pathology,in which there was 29 cases with hepatocellular carcinoma and 21 cases with hepatic hemangioma.Then the results were compared with conventional ultrasound and pathology.Results The CEUS manifestation of 96.6% patients with hepatocellular carcinoma showed swift enhancement in the arterial phase and swift expurgation in the portal phase,91.5% patients with hepatic hemangioma showed peripheral circular enhancement in arterial phase,centripetal filling in portal phase and hypoenhancement in delayed phase.The sensitivity,specificity and accuracy of CEUS in diagnosing were 96.6%,91.5% and 94.3%,respectively.Conclusion CEUS has higher clinical value than conventional ultrasound in differential diagnosis of hepatocellular carcinoma and hepatic hemangioma compared with common ultrasound.The characteristics of blood flow perfusion in CEUS are different between patients with hepatocellular carcinoma and hepatic hemangioma.
