The effects of short-term intensive treatment with insulin pump on levels of inflammatory factors in newly diagnosed type 2 diabetes

Abstract

Objective To observe the inflammatory state in newly diagnosed type 2 diabetes(T2DM) patients and the effect of short-term continuous subcutaneous insulin injection(CSII) therapy on inflammatory factors level.Methods Sixty cases of newly diagnosed T2DM(DM goup)received CSII therapy for two weeks,and 50 baseline data-matched healthy subjects were selected as control group.Inflammatory factors of pre-and post-treatment including interleukin 6(IL-6),tumor necrosis factor-α(TNF-α) and C-reactive protein(CRP) concentrations were examined,and insulin resistance indexes(HOMAIR) with homeostasis model were simultaneously determined in patients with T2DM.Results(1) The levels of IL-6,TNFα,CRP were significantly higher(65.67 ±14.50) vs(40.95 ±11.30) ng/L,P 0.01;(80.35 ±12.35) vs(60.08 ± 10.53) ng/L,P 0.05;(7.45 ± 0.85) vs(6.32 ± 0.53) ng/L,P 0.05;respectivelyin DM group than those in control group.(2) There were significant positive correlations between IL-6(r=0.6071,P 0.05),TNF-α(r=0.8033,P 0.01),CRP(r=0.7435,P 0.01) and HOMA-IR.(3) After 2 weeks CSII treatment,IL-6(50.67 ± 11.50) vs(65.67 ± 14.50) ng/L,P 0.05,TNF-α(68.33 ± 10.54) vs(80.35 ± 12.35) ng/L,P 0.05,CRP(5.58 ± 0.63) vs(7.45 ± 0.85) mg/L,P 0.05and HOMA-IR(6.50 ± 1.80)% vs(8.90 ± 3.70)% ,P 0.01were all significantly decreased than those before treatment.Conclusions Newly diagnosed T2DM patients were in a state of chronic inflammation,and the inflammatory state was correlated with formation of insulin resistance.Short-term CSII therapy in addition to lowering blood glucose can significantly reduce inflammatory factors,ameliorate the role of inflammation,reduce insulin resistance in patients with T2DM.
