Abstract
【Objective】To evaluate the clinical curative effects of small bone window craniotomy in the treatment of hypertensive cerebral hemorrhage (HCH). 【Methods】According to the different operative types,106 patients with hypertensive cerebral hemorrhage (HCH) were randomly divided into two groups:small bone window craniotomy group (n =66) and big bone flap craniotomy group (n =40). The indexes including duration of consciousness recovery,neurologic impairment score,total effective rate and mortality were comparatively analyzed. 【Results】The duration of consciousness recovery in small bone window craniotomy group was shorter than that in big bone flap craniotomy group (t =2.36,P 0.01). At postoperative 1 month,the variance of neurologic impairment score in small bone window craniotomy group excelled that in big bone flap craniotomy group (t=3.79,P 0.01). The total effective rate in small bone window craniotomy group was higher than that in big bone flap craniotomy group while the mortality in the former was lower than that in the latter (χ2=4.84,P 0.05). 【Conclusion】Small bone window craniotomy can be applied in the treatment of hypertensive cerebral hemorrhage with advantages such as reliable curative effect,slight trauma and few complications. It profits neurofunctional recovery especially in treating brain hemorrhage of medium capacity.
