External irradiation combined with intracavitary radiotherapy and concomitant chemotherapy for local adcanced esophageal carcinoma.

Objective To analyze the effect of external irradiation combined with high dose rate(HDR)brachytherapy and concomitant chemotherapy for local advanced esophageal carcinoma.Methods From Dec 1995 to Dec 1997,88 patients with local advanced esophageal carcinoma were treated.They were randomly divided into combined group(44 cases) and control group (44 cases).The combined treatment consisted of external radiotherapy to a total dose of 5520～6000CGY/46～50Fraction/23～25days and brachytherapy to a total dose of 500～1000CGy/2～4 fraction.The chemotherapy regimen was a combination of cisplatinum(DDP)80～100mg/m 2 d1 and fluorouacil (5 Fu)600～700mg/m 2 d 3-7 ,The concomitant chemotherapy was carried out on days 1～7 and 29～35.The controls were treated by external radintherapy and brachytherapy without chemotherapy.Results The 1 ,2 ,3 year survival rate was 79.2%,33.4%,15.6% in the combined group respectively and 77.3%,36.4%,12.5% in control group respectively (P=0.7049).The difference of residual or recurrent cancer and metastasis in both groups was similar.The motality due to complications was statistically different between the two groups(P0.05).Conclusions The external irradiation combined with brachytherapy and concomitant chemotherapy for local advanced esophageal carcinoma doesn′t improve 3 year survival rates.
