Thinprep cytologic test in combination with electronic colposcopy-guided cervical biopsy in the diagnosis of cervical disease.

Abstract

Objective To evaluate the clinical value of the application of thinprep cytologic test(TCT) combined with electronic colposcopy-guided cervical biopsy in the diagnosis of cervical disease.Methods Screening for cervical lesions was performed in 3865 cases during November 2007-October 2009.All patients underwent TCT test and electronic colposcopy classified with TBS classification standards.Of the 3865 cases,385 cases with positive TCT or clinically suspicious cervical lesions were further examined with colposcopy-guided cervical biopsy.Results TCT was unsatisfactory in 42 specimens(1.11%).Of the rest 3823 cases,negative for intraepithelial lesion or malignancy(NILM) with or without inflammation were observed in 3525 cases(91.2%).Positive detection rate of abnormal cytology was 7.71%(298/3865) including 136(45.6%) atypical squamous cells of undetermined significance(ASCUS),9(3.02%) atypical squamous cells-cannot exclude high-grade squamous intra-epithelial lesion(ASC-H),112(37.6%) low-grade squamous intraepithelial lesion(LSIL),34(11.4%) high-grade squamous intraepithelial lesion(HSIL),6(2.01%) squamous cell carcinoma(SCC) and 1(0.34%) atypical glandular cells(AGC).Of the 385 cases who underwent colposcopic biopsy examinations,pathological diagnosis was as follows: 189(49.2) chronic cervicitis,103(26.7%) cervical intraepithelial neoplasia I(CIN I),48(12.5%) cervical intraepithelial neoplasia Ⅱ(CIN Ⅱ),36(9.35%) cervical intraepithelial neoplasia Ⅲ(CIN Ⅲ) and 9(2.33%) SCC.Conclusion TCT examination may reflect cervical lesions,the false negative findings in TCT test indicate that TCT should be combined with electronic colposcopy-guided cervical biopsy to further improve the detection rate of cervical disease.
