Thoughts about Strategies for Chronic Disease Integrated Control on the Basis of Community Health Management

Based on the analysis of the disease prevention status quo and problems,and using community health management theory,new strategies are put forward for chronic disease prevention on the basis of community health management,including strategy for improving subjects’ and receptors’ initiative and enthusiasm to chronic disease prevention,strategy for community residents’ individual electronic health archives management and utilization,and strategy for phased intervention packet of chronic disease prevention interventions,to improve effect of chronic disease prevention strategy and the community residents’ health and life quality.
