Abstract
Objective To evaluate the efficacy and safety of trabeculectomy for primary acute angle-closure glaucoma with high intraocular pressure(IOP).Methods Retrospective analyzed 53 cases(56eyes)of primary acute angle-closure glaucoma,which underwent trabeculectomy.These cases were divided into 2 groups by the preoperational IOP:the high IOP group(15 eyes,IOP≥40mmHg),and the control group(41 eyes,IO P 40mmHg).And analyzed the IOP,visual acuity,complications and surgery outcome of both groups.Results Among all of 56 eyes,hypotony and hypotony maculopathy were observed in 9 eyes without significant difference( P 0 05).No severe complications such as choroidal hemorrhage,intraocular hemorrhage and malignant glaucoma were observed in all cases during surgery and postoperatively.The success rate of control group was higher than the high IOP group( P =0 0331)in the near future.When combined with laser suture lysis,eyeball massage,bleb needling and 5-Fu subconjunctival injection,the success rate of both groups improved and there were not significant difference between them( P 0 05).The postoperative visual acuity of high IOP group improved,and most of the patients maintained a better vision.Conclusions Trabeculectomy for primary acute angle-closure glaucoma with persistent high intraocular pressure is safe and effective.To keep the visual function from more damage,trabeculectomy should be performed for these patients actively.
