Clinical comparison of the effect on patient controlled intravenous analgesia and patient controlled epidural analgesia for postoperative analgesia

Abstract
Objective:To compare the clinical effect,safety and the side effects of patient controlled intravenous analgesia(PCIA) with those of patient controlled epidural analgesia(PCEA) for postoperative analgesia.Methods:90 postoperative patients were randomly divided into three groups,group PCIA,group PCEA and group control,30 cases for each.The patient controlled analgesia(PCA)pump system of compound of morphin heloperidolum(or droperidolum)was used via both of PCIA and PCEA postoperatively at once.But analgesic medicine was no given to the group control.The visual analgesia score(VAS)and the scale of sedation at {1 h},{4 h},{8 h},{24 h},{48 h} were reviewed and recorded accordingly after operation.The items recorded including recovery of bowel movement,urinary retention,recovery of uterus,postpartum hemorrhage,nausea,vomiting,pruritis and respiratory depression were observed.Results:Overall patients in two PCA groups were satisfied with the postoperative analgesia,The VAS scores at every correspording time intervals were significantly lower than those of the control group(P0.01).Recovery of bowel movement:The time were no remarkable differeness among the three groups (P0.05).The Incidence of urinary retention:PCIA was significantly lower than PCEA (P0.05).There were no remarkable difference among the three groups in effects of recovery of uterus,postpartum hemorrhage,nausea,vormting and pruritus (P0.05).For respiratory and circulatory depression PCEA was a litter higher than that in PCIA,but their difference was of no significance(P0.05).Conclusions:Both PCIA and PCEA have excellent analgesic effect.PCIA has no incidence of urinary retention and has no side effect induced in respiratory and circulatory system.Analgesic quality in group PCIA was superior to group PCEA.
