The therapeutic effect of spironolactone at different dosage on congestive heart failure

Objective To observe the therapeutic effect and safety of spironolactone at different dosage on congestive heart failure(CHF).Methods Totally 110 cases with CHF were randomized divided into low dose spironolactone treatment group(54 cases) and large dose spironolactone group(56 cases).Cardiac function parameters and adverse reaction were observed and compared before and 6 months after treatment.Results After 6months,the LVEF,LVMI,LVEDVI and LVESVI were 0.41±0.08,(128±23)g/m~2,(95±25)ml/m~2 and(54±20)ml/m~2 in large dose spironolactone treatment group,and 0.36±0.09,(140±26) g/m~2,(107±27 ) ml/m~2 and(65±22) ml/ m~2 in low dose spironolactone treatment group.All cardiac function indexes were improved compared with baseline in both two groups(P0.05 or P0.01).Furthermore,the therapeutic effects were more obviously in larger dose spironolactone group than low dose spironolactone group(P0.05).There were no severity adverse reactions in both two groups.Conclusion It demonstrated that treatment of CHF with large dose spironolactone would be effective and safety.
