Comparison of early results after pylorus-preserving pancreatoduodenectomy and Whipple resection

Objective To compare the early postoperative results of pylorus-preserving pancreato duodenectomy with Whipple resection. Methods The data of 123 consecutive patients who underwent pancreatoduodenectomy from Dec. 1994 to Dec. 2003 were analyzed retrospectively. Results Compared with Whipple, pylorus-preserving pancreato-duodenectomy results in a higher incidence of delayed gastric emptying, more nasogastric tube drainage days, an increased volume of gastric drainage and decreased volume of biliary drainage during the first 3 postoperative days, but shorter operation time and hospital stay. Conclusion According to early postoperative results, pylorus-preserving pancreato-duodenectomy is a better choice for periampullary tumor, especially benign tumor.
