Abstract
Objective:TO study the value of mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean corpuscular hemoglobin concentration(MCHC),red blood cell volume distribution width(RDW),red cell osmotic fragility test(RCOF) and hemoglobin electrophoresis for the diagnosis of thalassemia and iron deficiency anemia(IDA).Methods:The control group consisted of 30 people,the subjects group consisted of 60 patients with α-thalassemia,40 patients with β-thalassemia,30 patients with IDA.All samples were detected by complete blood count, red cell osmotic fragility,Hb electrophoresis and genetic test of thalassemia.Results:In subjects group,the values of HbA,HbA2,HbF were abnormal,and MCV,RCOF decreased,which were significantly different from the control group(P0.05).The sensitivity and specificity to thalassemia of single test were as follow: those of MCV were 98% and 93%,red cell osmotic fragility were 77% and 80%,the sensitivity of HBA2 toαthalassemia and βthalassemia were 83.3%、95%,respectively.The sensitivity of RDW was73%,similar to the control group,but significantly higher than that in people with IDA,the sensitivity were 70%.Conclusion:MCV,RCOF and Hb electrophoresis were reliable in screening thalassemia.RDW is recommended for screening IDA before the measurement of serum ferritin.
