Abstract
Objective:To evaluate the clinical value of serum troponin I(cTnI) determination in unstable angina pectoris(UAP).Methods:In 80 UAP patients,solid\|phase immunochromatographicassay and enzymicassay were adopted to measure serum cTnI,creatine kinase(CK)and isoenzyme MB(CK\|MB) respectively,Braunwald classification was adopted, and the in\|hospital incidences of cardiac events were investigated. Results:Among 27 5% of the UAP patients cTnI levels were weak\|positive or positive, while only 2 5% of them CK\|MB levels were increased( P =0 00001).The proportion of Braunwald class of patients in cTnI positive group(22 cases) was significantly greater than that in cTnI negative group(58 cases),it was 77 3% vs 48 3%, P =0 02.The in\|hospital incidences of cardiac events were significantly higher in cTnI positive group than those in cTnI negative group(40 9% vs 6 9%, P =0 007).Multivariate logistic regression analyses revealed that the determination of serum cTnI was most mainly independent predictor of the in\|hospital cardiac events. Conclusion:Serum cTnI is a more sensitive and specific marker of myocardial injure,it is most mainly independent predictor of the short\|term prognosis of patients with unstable angina.
