The Value of Cho/NAA Residual Z-score from 2D1HMRS for Grading in Astrocytoma

Abstract

Objective To discuss the value of Cho/NAA residual Z-score for histological grading of astrocytoma.Materials and Methods Subpeak area of NAA、Cho、Cr/PCr、Lac、Lip were calculated from all the healthy volunters and patients with astrocytoma. The value of the Cho/NAA、Cho/Cr from the control voxels and the value of Z-score、maximal Z-score from the excluded voxels in each subject were obtained.Results There were significant difference in the maximal Z-scores between the grade-Ⅲ astrocytoma and grade-Ⅰ、Ⅱ astrocytoma, between grade-Ⅲ astrocytoma and grade-Ⅳ astrocytoma (P0.05). Abnormal lactate peak was exhibited in 7 cases of astrocytomas (n=26), 1 case of grade-Ⅰ、Ⅱ astrocytomas (n=10), 2 cases of grade-Ⅲ astrocytomas (n=8), 4 cases of grade-Ⅳ astrocytomas (n=8) respectively. Lip peak was exhibited in 7 cases of astrocytomas (n=26), no case of grade-Ⅰ、Ⅱ astrocytomas, 2 cases of grade-Ⅲ astrocytomas (n=8), 5 cases of grade-Ⅳ astrocytomas (n=8) respectively.Conclusion Combination of maximal Z-score and Lac、Lip can provide the information for grading of astrocytoma. The maximal Z-score was higher in grade-Ⅲ astrocytoma than that of grade-Ⅰ、Ⅱ astrocytomas. Lowest maximal Z-score with Lac、Lip suggests grade-Ⅳ astrocytoma with necrosis.
