Abstract
Objective To explore the characteristics of CT hemoperfusion for breast cancer and its clinical value.Methods Twenty-two patients with breast cancer and 11 patients with benign lesions were all proven by surgery or biopsy.Using a spiral plus 16(Light Speed)scanner,an plain scan was performed through the anatomical region in question.Then sections through the tumor were selected.Using an injector pump,nonionic contrast agent was injected via cubital vein at a rate of 3.5 ml/s,and 5 s delay,then a multisection perfusion procedure was performed with a cine mode up to 50 s.The parameters of hemoperfusion such as blood flow(BF),blood volume(BV),mean transit time(MTT),and permeability surface area product(PS)in tumor peripheral zone,central portions,and contralateral normal breast tissue were acquired by using a software package.Seven patients who received chemotherapy underwent the second examination and measurement within a week after their first therapy.The values of hemoperfusion were analyzed statistically.Results The median of BF,BV,MTF,and PS in malignant tumor and in benign lesions were 31.78 ml·min~(-1)·100 g~(-1),6.98 ml·100 g~(-1),12.13 s,16.34 ml·min~(-1)·100 g~(-1),and 10.45 ml·min~(-1)·100 g~(-1),2.02 ml·100 g~(-1),15.49 s,3.33 ml·min~(-1)·100 g~(-1),respectively.The median of BF,BV,MTT,and PS in the central portions of malignant tumor were 23.07 ml·min~(-1)· 100 g~(-1),2.43 ml·100 g~(-1),11.77 s,and 5.99 ml·min~(-1)·100 g~(-1),respectively.The median of the difference of BF,BV,MTT,and PS before and after therapy were 15.02 ml·min~(-1)·100 g~(-1),1.93 ml· 100 g~(-1),1.98 s,and 8.01 ml·min~(-1).100 g~(-1),respectively.The difference of BF,BV,and PS between breast cancer and normal breast tissue,between peripheral zone and central portions of tumor,and between pre-therapy and post-therapy of breast cancer were statistically significant(P0.05).However,the difference of MTT between breast cancer and normal breast tissue,between peripheral zone and central portions of tumor,and between pre-therapy and post-therapy of breast cancer were not statistically significant (P0.05).Conclusion CT hemoperfusion is an indirect reflection of biological behavior of breast cancer and may has important value for the diagnosis,prognosis,and therapy monitoring of breast cancer.
