Abstract

Objective To clarify whether adolescent myopes have greater accommodative lag under monocular and binocular conditions,and to see if there is a relationship between myopia progression and accommodative lag.Methods Fifty-six juveniles without ocular abnormalities were divided into 3 group based on the following classifications: stable myopes(n=20),progressing myopes(n=21) and emmetropes(n=15).The subjects were lenses to correct their refractive errors.Accommodative responses under two stimuli(20 cm,35 cm from their eyes) were measured under monocular and binocular conditions using a retinoscope with a semi-reflective mirror.Results ①The accommodative lag of progressing myopes was greater than for stable myopes and emmtropes(P0.05),the accommodative lag did not significantly differ between the stable myopes and emmetropes(P0.05).②The accommodative response under binocular conditions was greater than for that under monocular conditions,and the binocular accommodative lag was significantly smaller than the monocular accommodative lag(P0.01).③The accommodative lag was greater in response to 20 cm distance targets than to 35 cm distance targets(P0.01).Conclusion The large accommodative lag may play a significant role in myopia development and progression.The increased accommodative demand leads to increased lag.The accommodative response was more accurate under binocular conditions than under monocular conditions.
