Analysis of clinicopathological feature of hepatitis B virus associated glomerulonephritis in 30 children

Abstract

Objective To investigate the clinical and pathological features of hepatitis B virus associated glomerulonephritis(HBV-GN) in children.Methods A retrospective study of 30 children with HBV-GN confirmed by renal biopsy from 2000.3 to 2005.3 was conducted.Clinical and pathological features of these cases were analyzed under the light microscope and immunohistochemical methods.Results The average age of initial onset was 8.4±2.9 years.The ratio of boys to girls was 9.The clinical manifestation included nephrotic syndrome in 25 cases(83.3%),isolated hematuria in 2(6.7%),isolated proteinuria in 2(6.7%) and nephritic syndrome in 1(3.3%).The pathological classification included membranous nephropathy(MN) in 18 cases(60%),mesangial proliferative glomerulonephritis(MsPGN)in 8(26.7%),membranoproliferative glomerulonephritis in 3(26.7%) and endocapilary proliferative glomerulonephritis(EnPGN) in 1(3.3%).Immunohisochmical results showed multiple immune compound deposits.Hepatitis B viral antigens was detected in the glomerular deposits from 22 cases(73.3%).Hepatitis B viral antigens and immune compound deposited in the same localization in glomeruli.Conclusion HBV-GN is common in school-aged children,boys are far more than girls.the predominant presenting feature is nephrotitic syndrome.The renal biopsy showes multiple pathological classifications,mainly are membraneous nephropathy.The clinical and pathological features of HBV-GN indicates its pathogenesis.
