Abstract
Objective To explore the effects of noninvasive ventilation on pulmonary gas exchange during acute hypercapnic exacerbations of chronic obstructive pulmonary disease.Methods 57 COPD patients with acute severe hypercapnic respiratory failure were randomly divided experimental group(21 cases)and control group(31 cases).Measurements of pulmonary gas exchange were done:(1)breathing spontaneously(baseline); (2) after 2 h and 5 days of BiPAP using a full face mask.Results The experimental group were significantly improvement in PaO 2,a fall in PaCO 2 and in pH and HCO 2 - after 2 h and 5 days by BiPAP(P0.05).The heart rate and breath rate were significantly decrease after 2 h of BiPAP(P0.05).The side effect was limited.Conclusion BiPAP could be proposed as a effective method in COPD patients with Ⅱ type respiratory failure.
