Practice and Exploration of Family Doctor Service Model Based on Health Management for Focus Groups in Ningbo Jiangdong District

This research defined the meaning of health management at the present stage,cleared health management work content and work flow of family doctors,described the health management support system,in order to improve the connotation of health management of family doctors. The measures included were implementing periodic health examination,establishing and improving health records,assessing health risk,performing management of hierarchical refinement,giving health tracking and tips,opening the hotline,guiding the way of life,practicing targeted control management process during the whole course,building up the network of health management,developing health examination information system of health management,establishing the support system of family physicians training based on health management concept. In the end,the health cognition of focus groups was upgraded,and the level of health management was increased.
