OBJECTIVE To explore characteristics of the distribution and drug resistance of pathogenic bacteria causing urinary tract infections in the senile patients with type 2 diabetes and analyze the risk factors of the urinary tract infections.METHODS A total of 74 patients with urinary system infections were selected from 288 elderly patients with type 2diabetes,then the urine specimens of the patients were collected to perform the bacterial culture and the drug susceptibility testing;the univariate and multivariate logistic regression analysis were performed to determine the risk factors including the gender,age,blood glucose,glycosylated hemoglobin,complication,and invasive operation.RESULTS Of totally 102 strains of pathogens isolated from 74 patients with urinary tract infections,there were 66(64.71%)strains of gram-negative bacteria and 36(35.29%)strains of gram-positive bacteria.Escherichia coli,Staphylococcus epidermidis,Enterococcus,Proteus,and Klebsiella pneumoniae ranked the top species of pathogens,accounting for 40.19%,20.59%,12.75%,9.80%,and 5.88%,respectively.The drug susceptibility rates of the gram-negative bacteria to imipenem and meropenem were more than 97.0%,the gram-positive bacteria were highly sensitive to teicoplanin,and no strains resistant to vancomycin were detected.More than 70years of age,gender of female,course of diabetes more than 10 years,invasive operation,glycosylated hemoglobin more than 8%,fasting plasma glucose more than 13.9mmol/L,and length of hospital stay more than one month were the risk factors of urinary system infections in the elderly patients with type 2 diabetes.CONCLUSION The risk factors should be intervened during the treatment of urinary system infections in the elderly patients with type 2 diabetes,and the bacterial culture of the urine should be carried out so as to choose proper antibiotics.
