New electrocardiographic criteria for predicting either the right coronary arterry or left circumflex artery occlusion

Abstract

To find the electrocardiography (ECG) characteristic for predicting either the right coronary artery (RCA) or left circumflex artery (LCX) occlusion by comparative analysis of standard 12 lead ECG and selective coronary cineangiography (CAG).Method:56 patients with inferior wall acute myocardial infarction (AMI) who were admitted to coronary care unit were performed ECG and CAG.Criteria A Ⅲ st↑ Ⅱ st↑ 、B(aVL st↓ Ⅰ st↓ )、C(A and B positive)and D (A and B negative) were analysed for predicting either RCA or LCX occlusion.Result:The prevalence of criteria A、B、C in RCA occlusion group were significantly higher than in LCX occlusion group (P 0.001 ).The prevalence of criteria D in LCX occlusion group were higher than in RCA occlusion group (P 0.001 ).The sensitivity of criteria A、B、C for predicting RCA occlusion were 74%,77%,72%,the specificity 94%,88%,100%,respectively.The sensitivety,specificity positive predictive value and negative predictive value of criteria D for LCX occlusion were 82%,97%,93% and 93% respectively.Conclusion:The criteria A、B、C、D in ECG could aid to clinic diagnosis of RCA or LCX occlusion.
