Curative Effect of Comprehensive Intervention on First-episode Depression and Changes of Serum Homocysteine and Hypersensitive C-reactive Protein

Objective To observe the curative effect of comprehensive intervention( Mirtazapine combined with psychological nursing) on first- episode depression and the level changes of serum homocysteine( Hcy) and hypersensitive C- reactive protein( hs- CRP),and to explore the clinical value of monitoring. Methods 90 patients with first- episode depression were randomly divided into treatment observation group( 50 cases) and treatment control group( 40 cases),both groups were given Mirtazapine( 30mg / d) treatment,on the basis,treatment observation group combined with psychological nursing intervention,two groups were continuously observed 8 weeks of treatment. 40 cases healthy were chosen as normal control group during the same period. The symptoms of depression was assessed with Hamilton Depression Rating Scale( HAMD),and the level of serum Hcy and hs- CRP were detected,curative effect compared. Results After treatment for 8 weeks,HAMD score and the levels of serum Hcy and hs- CRP of treatment observation group were lower than that of treatment control group,the differences were statistically significant( P 0. 01); The levels of serum Hcy and hs- CRP of treatment observation group compared with normal control group,there was no statistically significant difference( P 0. 05); The levels of serum Hcy and hs- CRP of treatment control group were higher than that of normal control group,the differences were statistically significant( P 0. 01).After 8 weeks,the effective rate of treatment observation group was 94. 0%,which higher than that of control group of 75. 0%,the difference was statistically significant( P 0. 01); The HAMD scores of depression was positively correlated with serum Hcy and hs- CRP( r = 0. 602,0. 597,P 0. 01). Conclusion Patients with depression exists high Hcy and hs- CRP,Mirtazapine combined with psychological nursing intervention can significantly improve the level of Hcy and hs- CRP and improve clinical curative effect,the detection of serum Hcy and hs- CRP level can be estimated from depression patients,observe the clinical curative effect and prognostic indicators.
