Clinical research on bilevel noninvasive positive pressure ventilation in the treatment of patients with chronic obstructive pulmonary disease and type II respiratory failure

Abstract

Objective To investigate the clinical efficacy of bilevel noninvasive positive pressure ventilation(BiPAP) in the treatment of patients with chronic obstructive pulmonary disease(COPD) and type Ⅱ respiratory failure.Methods 58 cases of COPD patients with type Ⅱ respiratory failure selected in the general hospital of Shenyang military region from May 2008 to October 2011 were divided into two groups randomly.29 cases who received conventional treatment were defined as the control group,and the 29 cases who received conventional treatment combined with BiPAP were defined as the observation group.The clinical indexes were compared between the two groups after 24 h treatment.Results Compared with before treatment,the respiratory frequency,heart rate and arterial carbon dioxide partial pressure(PaCO2) in the control group and the observation group decreased significantly after treatment,while the oxygen saturation(SpO2) and arterial oxygen partial pressure(PaO2) increased significantly after treatment(all P 0.05).Therefore,the respiratory frequency,heart rate and PaCO2 in the observation group were significantly lower than those in the control group after treatment,while the SpO2 and PaO2 were significantly higher than those in the control group after treatment(all P 0.05).Moreover,the total effective rate in the observation group was significantly higher than that in the control group(all P 0.05).Conclusion The BiPAP in the treatment of COPD patients with type Ⅱrespiratory failure can improve the oxygenation function markedly,which not only avoids tracheal intubation,but also reduces the mortality rate and improves the quality of life,so the BiPAP is a safe and effective treatment method.
