Relationship of insulin resistance with extent of coronary artery disease in patients with stable angina pectoris

Abstract
Objective To explore the relationship of insulin resistance with coronary artery disease. Methods A total of 272 patients with stable angina pectoris (SAP) to have coronary angiography were enrolled in the present study,of which 199 were males; the patients had a mean age of (59±8) years old. Patients with diabetes were excluded from the study. Biochemical indices such as fasting blood insulin concentration,blood lipids and blood glucose were all determined. The extent of insulin resistance and coronary artery disease were evaluated by McAuley index and coronary score,respectively. Results Eighty-five patients (31.3%) were found to have insulin resistance,76 (27.9%) had no coronary atherosclerosis,78 (28.7%) had coronary atherosclerosis but with stenosis 50% and 118(43.4%) of the 272 patients had stenosis ≥50%. McAuley index had a significant negative correlation with coronary score (r=-0.153,P0.05),while fasting insulin concentration had no significant correlation with coronary score (P0.05). Patients were divided into 3 groups according to coronary score (group A 1.9,group B 1.9-4.83,group C≥4.84) by K-means clustering. The McAuley index in group C was lower than that in group A (P0.05). Fasting insulin concentration showed no significant differences among 3 groups (P0.05). Conclusion The extent of insulin resistance is positively correlated with the extent of coronary artery disease as evaluated by coronary angiography,while fasting insulin concentration has no significant correlation with extent of coronary artery diesease.
