Abstract
[Objective]To explore early appropriate fluid resuscitation for severe acute pancreatitis patients.[Methods]From January 2009 to May 2011,36 patients with SAP in our hospital were randomly divided into control group and observation group.Based on the conventional treatment method,the control group was used with rapid,large-volume fluid resuscitation,the observation group was treated with appropriate fluid resuscitation.Compared the average time of expansion up to standard,infusion speed,fluid retention,biochemical index,total complication rate and prognosis of patients in two groups.[Results]In the control group,the fluid expansion time and HCT of patients were significantly lower than in the observation group(P﹤0.05);the average speed of infusion,fluid retention volume were significantly higher in the control group than in the observation group(P﹤0.05);serum BUN,SCr,AST,ALT,TBiL and AMY of patients in the observation group after treatment were significantly decreased than in the control group,and the arterial blood oxygen pressure(PaO2)increased significantly,both with significant difference(P﹤0.05);the mechanical ventilation rate,APACHE II score,abdominal compartment syndrome,heart failure rate,MODS rate,mortality rate were significantly higher in the contrl group than in the observation group(P﹤0.05),the intra-abdominal pressure in the control group was significantly higher than that in the observation group(P﹤0.05).[Conclusion]The early appropriate fluid resuscitation may be effective in restoration of circulating blood volume,preventing fluid retention,which can significantly improve the cure rate,is worth popularizing.
