Abstract
Objective To assess clinical efficacy of fore-course hyperfractionated and late-course hyperfractionated accelerated radiotherapy in the treatment of esophageal carcinoma.Methods The fore-course hyperfractionated radiotherapy was scheduledas 110 cGy/fraction,2 times/day,10 fractions/week with a total dose of 30-40 Gy/2.5-3.5 weeks,then the late-course hyperfractionated accelerated radiotherapy was scheduled as 140-150 cGy/fraction,2 times/day with an interval of 6 hours between the two fractions,10 fractions/week with a total dose of 30 Gy/2 weeks. In the whole course of the treatment, the patients were given a total dose of 60-70 Gy/4.5-5 weeks.Meanwhile, the other patients with esophageal carcinoma who accepted routine radiotherapy belonged to the control group.Results The effective rate and 1-year survival rate of fore-course hyperfractionated and late-course hyperfractionated accelerated radiotherapy group (study group)and routine radiotherapy group (control group) were 83.8%,70.9% and 70.0%,46.7 % respectively, There was significant difference between the two groups(P0.05).However, acute radiation-induced esophagitis in study group was more severe than that in control group,but the esophagitis was tolerable in study group.Conclusion Compared with routine radiotherapy, fore-course hyperfractionated and late-course hyperfractionated accelerated radiotherapy for treatment of esophageal carcinoma has the advantages of shorter treatment course, higher efficacy, obviously enhancing local control rate of esophageal carcinoma and 1-year survival rate of the patients and tolerable side-effect.
