Comparative study on the therapeutic efficacy of repaglinide versus acarbose in treatment of type 2 diabetes mellitus patients

Abstract
Objective To compare clinical therapeutic efficacy of the repaglinide and acarbose in the on type 2 diabetes mellius patients. Methods Sixty six patients with type 2 diabetes mellitus,were randomly divided into two groups and treated for 12 weeks either with repaglinide(n=34) or acarbose(n=32),respectively . The values of FPG, P2hBG, A1C, FINS, P2hINS, blood-fat and BMI were periodically measured and compared before and after treatment. Results The concentrations of FPG decreased both in rapaglinide group and acarbose group (the averages value were 3.52mmol/L and 2.89mmol/L respectively (P0.01). Both drugs could reduce the value of P2hBG significantly (P0.01),which had similar effects as well. The levels of A1C were significantly decreased in two groups, the concentrations of A1C reduced from 9.23±1.32)% to(7.04±0.57)% (P0.01) in rapalinide group, and from (9.37±1.24)% to (7.08±0.63)% (P0.01) in acarbose group, but without statistically significant difference (P0.05).The values of P2hINS were significantly increased (P0.01) as compared with those in rapalinide group before treatment, but were significantly decreased (P0.01) in acarbose group After treatment, BMI was slightly increased by 1.3% (P0.05) in rapalinide group and decreased by 2.7% (P0.05) in acarbose, showing significant difference between the two groups (P0.05). Conclusion The values of FPG, P2hBG and A1C were significantly decreased in both groups. repaglinide and acarbose are more suitable for treatment of patients whose value of P2hBG were increased apparently after diet. The effects of rapaglinide is better than acarboae in reduce FPG. Acabose has the effect of reducing postprandial insulin secretion, regulating blood-fat and losing body weight, thus is more suitable to obese type 2 diabetics.
