Evaluation of Spiral CT in the Diagnosis of Portal Vein Involvement

Abstract

Objective To assess the accuracy of spiral CT portography in evaluating the portal vein involvement.Materials and Methods Spiral CT portography was performed in 75 patients before hepatic surgery. A total of 225 portal veins, including the main, right and left portal veins, were assessed. An evaluation of portal vein involvement, such as encasement, occlusion and tumor thrombosis, was made. The diagnosis made by spiral CT was compared with the surgical and pathological results.Results Spiral CT portography detected 42 out of 43 involved veins, and confirmed 180 out of 182 non involved veins, with a sensitivity of 97.7%, specificity of 98.9%, positive predictive value of 95.5% and negative predictive value of 99.4%.Conclusion Spiral CT portography is quite accurate in evaluating portal vein involvement.
