Abstract
Objective : To investigate insulin sensitivity and androgen production in non-obese and non-insulin resistance (IR) women with polycystic ovary syndrome (PCOS) during treatment with Diane-35 (Estradiol+Cyproterone). Methods: 32 non-obese and non-IR PCOS women were enrolled into the study. After a baseline work including body mass index (BMI), waist to hip ratio (WHR), ovary volume, serum follicle stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), sex hormone-binding globulin (SHBG), Leptin, free androgen index (FAI), glucose, and insulin concentration during oral glucose tolerance test (OGTT), at the end of 3 months Diane-35 therapy, all subjects were re-evaluated. Results: ①BMI,WHR and FSH remained unaltered, significant decreases in concentration of LH, LH/FSH, FAI, ovary volume( P 0.01)and T( P 0.05), and an increase in concentration of SHBG and Leptin were found in these PCOS subjects. ② No change in fasting glucose and area under curve(AUC) of glucose in OGTT was observed, but AUC of insulin and insulin to glucose ratio in OGTT were increased significantly compared to pre-treatment, while fasting glucose to insulin ratio markedly decreased( P 0.01). Conclusions: The reduction in androgen concentrations by Diane-35 was accompanied by some extent deterioration in insulin sensitivity, probably associated with reduced peripheral tissue insulin sensitivity in these non-obese and non-IR PCOS subjects.
