Comparative Therapeutic Effect with Interventional Chemothermotherapy and Interventional Chemotherapy in the Treatment of Advanced Hepatocellular Cancer

Abstract
Objective To observe and compare the volumes and survival periods of tumors in the patients with advanced liver cancer after treatments of transhepatic arterial infusion and heat infusion.Methods There were totally 120 cases of advanced liver cancer from January 1996 to January 1999.In the first group,39 cases were treated with ordinary transhepatic arterial chemotherapy.The second group(40 cases)were treated with transhepatic arterial thermochemotherapy(60℃),the third group(41 cases) were treated with transhepatic arterial thermochemotherapy(60℃)with heated lipiodol(60℃)embolization.In all three groups transfemoral catheterization was applied,catheterial head was placed into tumors feeding artery(common hepatic artery,left or right hepatic artery,hepatic segmental artery),chemotherapic protocol was adriamycin(ADM)+mitomycin C(MMC)+hydroxycamptotheccin(HCPT),repeated treatment was given after 30~40 days and continued for two or three times.A week and a month before and after treatment,seral ALT,AST,BUN,Cr were examined,the volumes of tumor were measured on CT,US and DSA,patients survival periods were observed.Results (1)after first,sencod,third treatment,the volumes of tumor(cm) were measured respectively:1 group(7.9±0.84,7.1±0.44,6.6±0.23),2 group(9.2±1.21,7.4±0.46,5.9±0.75),3 group (10.6±2.1,8.9±0.57,6.1±0.43).(2)0.5,1,1.5,2 year survival rate:2 group (95.5%,74.3%,49.8%,44.5%),3 group (96.7%,85.2%,63.5%,52.7%),the survival rates of 1 group and 2 group were higher than the 1 group (91.2%,64.5%,23.4%,10.1%)(Ρ0.05).(3)3 day after treatment,levels of seral ALT,AST,Cr incresed,but after a month the levels were not different from that before treatment(Ρ0.05).Conclusion Transarterial thermochemotherapy with heated lipiodol embolization is more efficient than ordinary transarterial chemotherapy in controlling tumor and prolonging survival period of patient with advanced liver cancer,there are transient effects on function of liver and kidney only.Transarterial thermochemotherapy with heated embolization is a more efficient interventional method in treatment of advanced liver cancer.
