Effects of immune factors and clinical outcomes of xingnaojing injection therapy on patients with ventilator associated pneumonia

Abstract
Objective:To investigate the changes of serum levels of C reactive protein(CRP),interleukin-6(IL-6)and TNF-α(Tumor Necrosis Factor-α)of Xingnaojing therapy on patients with ventilator associated pneumonia(VAP)and the correlations between these immune factors and the time of mechanical ventilation(MV).From the investigation,we should explore the clinical outcomes of these patients with Xingnaojing therapy.Methods:61 patients with VAP were randomly divided into the therapy group(with basic therapy and Xingnaojing therapy)and control group(only with basic therapy).Levels of CRP,IL-6 and TNF-α were analyzed before the treatment and then analyzed again after1,3,5 and 7days of treatment.The changes of sternum before the treatment and after 3 days of treatment were compared.The times of MV and the probability of off-line with ventilation were statistics.Data were expressed as mean±standard and analyzed by using Analysis of Variance and correlation and linear regression.A P value less than 0.05 was regarded as statistically significance in all the tests conducted in this study,Results:Compared with the normal level,the serum levels of CRP,IL-6 and TNF-α in all 61 patients with VAP were increased.The CRP level of therapy group after 7 days of treatment was(31.31±11.52)mg/L,it was significantly decreased comparing with the level of control treatment group which was(56.09±16.82)mg/L(P 0.05).The IL-6 and TNF-α levels of therapy group after 5 days of treatment were(199.68±37.76)ng/L and(53.60±23.22)ng/L,began to decrease after 5 days of treatment,and they were decreased comparing with the level of the control treatment group which were(229.67±30.21)ng/L and(67.75±17.61)ng/L(P 0.05).Then the IL-6 and TNF-α level of therapy group were decreased significantly after 7 days of treatment which were(161.66±22.49)ng/L and(36.88±17.06)ng/L comparing with the level of normal treatment group which were(212.37±26.84)ng/L and(53.56±15.36)ng/L(P0.05).The time of MV of therapy group,which was(85.48±20.89)h,was shorter than control group,which was(100.75±28.70)h(P0.05).The improvement rate of sternum after 3 days with treatment of therapy group,which was 92.86%(26/28),and the probability of off-line with ventilation,which was 92.86%(26/28),were higher than control group(P 0.05).Levels of CRP,IL-6 and TNF-α after 7days of treatment were positively correlated with the time of MV.Conclusion:In combined therapy of VAP,the assist therapy of Xingnaojing injection regulates the inflammatory reaction obviously.It could restore the body's immune function and reduce the injury after VAP.Then,it could shorten the times of MV and improve the clinical outcomes.So we could use Xingnaojing in the therapy of VAP.
