Cataract extraction by non-phacoemulsification through a small-in- cision combined with trabeculectomy for the treatment of cataract complicated with glaucoma.

Abstract
Objective To evaluate the effectiveness of a 5.5 ~ 6.0 mm small-incision triple procedure, including cataract extraction by non-phacoemul-sification, posterior chamber intraocular lens implantation and trabeculec-tomy in patients with coexisting cataract and glaucoma. Methods The triple procedure was performed on 36 eyes of 30 patients with cataract and glaucoma through a small-incision by non-phacoemulsifieation.The postoperative follow-up was 3 to 36 months, the mean was 19.5 months. Results Decreased by 20.84 mmHg, the mean postoperative intraocular pressure was (15.38±3.78)mmHg(1 mmHg = 0.133 kPa)with an apparent difference from that before operation ( P 0.001 ) Postoperatively,the visual acuities were≥0.5 in 27 eyes (75.0%) at one week and in 30 eyes (83.3%) during 3-36 months.The mean astigmatism was (1.25 ±0.75) D one week afgter operation, increased by 0.25D compared with that before operation.The mean postoperative astigmatism at 3-36 months (1.15± 0.75) D was not distinctly different from the preoperative one ( P 0.05). By Kronfeld' s classification, the functional filtering blebs of type ⅠandⅡ were 26 eyes (72.2%) and type Ⅲwas 10 eyes (27.8%) .Conclusion It is effective to treat the patients with coexisting cataract and glaucoma through small-incision non-phacoemulsi fication triple procedure, with the postoperative visual acuity quickly restored,the astigmatism being low enough,the intraocular pressure well- controlled and postoperative complications was less.
