Abstract
Objective To study the distribution of biliary bacteria and the changes of their sensitivity to antibiotics ,we can use the result to provide a guide for rational administration of drug in clinic.Methods We examined 103 patients with positive bile specimens between January 2006 and June 2008 and devided into two groups according as the 159 strains of bacteria.One group is Community-acquired biliary infections and the other is Hospital-acquired biliary infection. We analyze the result in statistical.Results The 159 strains of bacteria contain 30 species:Gram negative bacteria accounted for 66% and Gram positive cocci accounted for 34%. The top 5 species of bacteria are Escherichia coli(31.5%),Enterococcus(25.8%),Pseudomonas(9.4%),Klebsiella pneumonia(8.2%) and streptococcus(6.3%). The species and proportion of rare pathogenic bacteria of the Hospital-acquired biliary infection group(32.1%) is significantly higher than that of the Community-acquired biliary infection group(17.9%). And the proportion of mixed infection of the Hospital-acquired biliary infection group (63.6%) is also significantly higer than that of the Community-acquired biliary infection group (30.5%). The sensitivity of bacteria to antibiotic including penicillin,cephalosporin,quinolone and macrolide decline widespreadly. And the sensitivity to Imipenem,Meropenem and teicoplanin is still high. The antibiotic tolerance of bacteria of the hospital acquired biliary tract infection group is markedly elevated.Conclusions The bacteria spectra of biliary tract infection varies in frequency,it also shows a general decline in the sensitivity of bacteria to antibiotics.Comparing within the two specimens,in species and types of infection,Hospital-acquired biliary infection is not only more complex than Community-acquired biliary infection,what's more,it also shows the antibotic resistance has become more serious.
