Abstract
Objective:To explore the blood gas changes and clinical significance of acid-base disturbance in patients with acute myocardial infarction (AMI). Methods: Parameter of blood gas (including pH,PaO2,PaCO2,HCO3-), types of acid-base disturbance and the results of electrolyte of 134 AMI patients were analyzed retrospectively. Results: PaO2 value was below 80 mmHg in 72 patients (53%), 113 patients (84%) showed different types of acid-base disturbance. The most common form was respiratory alkalosis plus metabolic acidosis, which was seen in 24 cases (18%). Others were respiratory acidosis plus metabolic acidosis, simple metabolic alkalosis, simple metabolic acidosis, and etc. There were 43 cases with simple acid-base disturbance (32%), 64 cases with double acid-base disturbance (48%) and 6 cases with triple acid-base disturbance (4%). Conclusions: Hypoxemia and acid-base disturbance are found commonly in patients with AMI, and is prone to complicated with metabolic acidosis when cardiogenic shock exist. Severe metabolic acidosis plus respiratory acidosis is a symbol of severe illness.
