Clinical experience of carotid endarterectomy for severe stenosis of internal carotid artery

Abstract

Objective To summarize clinical experience of carotid endarterectomy(CEA) for treating severe stenosis of internal carotid artery.Methods From Oct.1998 to Apr.2009,CEA was performed on 185 patents with internal carotid stenosis.In all cases,there were 115 males and 70 females.The average age was 66(range,51-86) years old.There were 117 cases with a history of transient ischemic attack and 21 cases with a history of cerebral infarction.All cases were diagnosed by selective angiography,which showed 70% stenosis of internal carotid artery was in all cases,among them,95% stenosis of internal carotid artery was found in 112 cases and contralateral carotid artery stenosis was in 35 of 112 cases.Moreover,86 cases were found coronary artery stenosis by coronary angiography.Simultaneously CEA and coronary artery bypass grafting(CABG) were performed on 25 of 86 cases.Results Postoperatively,clinical symptoms alleviated obviously in 146 cases.Two weeks after procedure,cerebral hemorrhage developed in 2 cases,of which 1 discharged after treatment and 1 died.And 1 case had hypoglossal nerve injury and 4 cases had the marginal mandibular branch of the facial nerve injury.No stroke occurred within postoperative 30-day.During the follow-up of 6-60 months,151 cases were followed up.All cases were well in addition to 1 case died of heart attack.Conclusion CEA is an effective and safe treatment for treating severe stenosis of internal carotid artery.
