Abstract
Objective:To investigate the role of C-reactive protein(CRP) and Plasminogen activators inhibitor 1(PAI-1) in the etiopathology of GDM and the correlation between inflammatory cykines and insulin resistance.(IR).Methods:The blood sample were collected from 29 gestational diabetes mellitus(GDM),31 impaired glucose tolerance(IGT) and 35 normal glucose tolerance(NGT).The levels of CRP,PAI-1 and glucose,insulin during 75g oral glucose tolerance test(OGTT) at 0,1,2,3h were detected by immunologic turbidimelry and radioimmunoassay.Insulin resistance was assessed by the homeostasis model insulin resistance index(HOMA-IR),Insulin secretion by the homeostasis β-cell function index(HOMA-HBCI).The difference in HOMA-IR,HOMA-HBCI,CRP and PAI-1 between three groups were compared and analysed.Results:①HOMA-IR was lower in NGT groups than in IGT groups and GDM groups,there was significant difference(P0.05) between NGT and IGT,NGT and GDM;HOMA-HBCI was higher in NGT groups than in IGT and GDM groups.There was significant difference(P0.05) between NGT and IGT,NGT and GDM;② Serum CRP and PAI-1 were higher in GDM than IGT,NGT groups.There was significant difference(P0.05) between NGT and IGT,NGT and GDM,IGT and GDM;③The level of CRP was correlated with FPG(r=0.331,P=0.01),HOMA-IR(r=0.303,P=0.035),and BMI(r=0.283,P=0.040);there was posistive correlation between surum concentrations of PAI-1and HOMA-IR(r=0.525,P=0.001),FIns(r=0.550,P=0.001),BMI(r=0.625,P=0.000).Conclusion:There is serious insulin resistance and insulin secretion decrease,which induce GDM together.There is positive correlation between CRP,PAI-1 and insulin resistance,it may play role in the etiopathology of gestational diabetes mellitus.
