The Household Population Health Management Model of Family Doctor Services Team in Zhangmutou Town

Objective To summarize and discuss the use of the family doctor service team of health management mode on household population in Zhangmutou town. Methods Using Zhangmutou town community health service centers in 29 communities in the town of mature family doctor service team according to health management to carry out the community health service project. Using low cost health project of Shenzhen Institute of advanced technology research institute,the resident health archives as carrier. Management process through the establishment,maintenance residents health records to the family doctor service team of community residents in the whole life of health and disease risk factors for detection, assessment and intervention. Results The residents health management coverage of more than 80%. Residents of chronic diseases was reduced by 30%. The residents average annual medical expenses reduced by 20%. The above indicators have been successf ully completed. Conclusion The use of universal low-cost health project of the Institute of the Chinese Academy of Sciences,Shenzhen,advanced technology,greatly reduced the cost of health management,and is conducive to the formation of a community health service institutions relying on new health management mode,all community residents inon the basis of universal access to basic health care, enjoy a higher level of health care services,and continue to improve the people's health and happiness index.
